The Trigger Effect

The series Connections 1 by James Burke has an opening episode called "The Trigger Effect" that discusses what happens when a technological system fails.

Here you can watch the first ten minutes of the "Trigger Effect" episode. At 5:41 on the video you can see the one small part that failed to cause the Northeast Blackout of 1965.

The UAL stock crash was a trigger effect incident.
